We appreciate the opportunity to participate in your patient's care. Our goal is to make the referral process as easy and efficient as possible for your office. We offer three options for submitting patient referrals:

1) New! Online Referral Form - Secure, online form for non-emergency referrals. We will contact the patient, schedule the appointment and then fax a confirmation back to your office. Please fax insurance cards and any applicable medical records to (614) 754-5601

2) Printable Referral Form (pdf) - for any referral. Please fax insurance cards and any applicable medical records to (614) 754-5601 and we will contact the patient to schedule the appointment.

For emergency referrals, please also call (614) 754-5600.

3) Call our scheduling department directly at (614) 754-5600. We will take the referral information over the phone, contact the patient, schedule the appointment and then fax a confirmation back to your office. Please fax insurance cards and any applicable medical records to (614) 754-5601
